New Issue w/ Forms - Default Date Values are getting messed up?

Hello,

For about a month or so I've been noticing that any default value dates entered into a smartsheet form are getting converted into something completely different. For instance, I have entered 2/27/2023 for today (screenshot 1)

But when I go to the spreadsheet, it's been changed to June 2 2023.

Other times it's been completely random, like some date in 2025.

This has been occurring across multiple sheets. Any fixes? Anyone else having this issue?

Thanks for the suggestions folks :) The column type is set to Date, and has been the same format across sheets I've used for a few months now (I just copy a new sheet as needed).

As for the activity log, the dates are being entered as the incorrect date:

The most recent second activity is me correcting the date entry.
