How to add a third option to Approval Workflow

Greetings!

Right now, an Approval Workflow includes two choices: Approved and Denied.

How do I create an approval workflow to include the following:

-Approved

-Approved with Conditions (and a place to type those conditions)

-Denied

You would need to use an Update Request instead.
