Collecting monthly availability from external contacts

Hello, 

I would like to set up a monthly notification to go out to candidates to obtain their availability.

I would have a list of contacts- the contact should receive a form in which they would be providing their availability to join a project. Let's say "available from April 15th to April 30th. This information should feed back to the list of contacts automatically as they respond. How can I achieve this?
