Can a form field check if the entered data already exists?

In a form, I want to present different fields based on the input the user entered in a previous field.

For example the user enters their license number in the license number field. If the license number already exists it presents a specific field if it doesn't exist the user is presented with different fields

    Here's a possible workaround or workarounds

    • Include the form in a dashboard side by side with a sheet with the license number or similar.

    Would that work/help?

    My other thought was to use a 'Temp Working Table' The user supplied information from the form would be put into this temporary sheet. The Column (License Number) in the Sheet (Temp Working Table) would check if the license existed anywhere in the Column (License Number) in Sheet (Broker Intake Sheet). If it did exist it would update the Column (Expiration Date) in the Sheet(Broker Intake Sheet) with the value from Column (License Expiration Date) in the Sheet (Temp Working Table). If it didn't exist it would copy values in different columns to a new row in the corresponding columns in the Sheet (Broker Intake Sheet). I'm struggling with the formulas.
