Form field is not pulling into sheet

One of the fields on my form is not pulling into the worksheet and I'm not sure why?

I've refreshed the document several times and its the only field that will not show up.

Any suggestions?

Are you able to provide screenshots of how the sheet is set up and how the form is set up?

Good news, I figured it out. Someone had hidden columns that I was not aware of.
