Has anyone used the widget generally assigned to the Project Intake Form for something else? Perhaps a custom fillable .pdf?

Thank you for your reply Andree. I've pasted a screen snap of the widget I was asking about. Right now, you can see this references the form that is associated with the Project Intake sheet. This widget is part of the Project Portfolio Roll-up dashboard. I am wondering if anyone has use a different form here - perhaps a custom .pdf file - or other type of project initiation/request form here?
