Hi, I am having trouble integrating the data from a form to a chart widget on a dashboard.

It is going to depend on how the data is structured and exactly how you want the chart to look. You can create a report that is grouped and then summarized, or you can use a metrics sheet, but either way, charts can only be created from numeric data.
