Editing the sheet

Hello, I am trying this sheet and really love the layout but it is not letting me edit the Phyician info sheet at all. I have tried everything I can think of. Any ideas?

I think we'd need a screen shot or additional description of the sheet to help more, as long as there is not PII or sensitive data.
