Updates to a sheet via form

I've seen several posts about updates to a sheet via a form or request an update via trigger.
Here is my task... Create a sheet to track mileage for a list of team member for reimbursement. Information on the sheet needs to stay confidential. I need to update three different cells on a team members record on the sheet. I've tried to send out a "Request to Update" but I can't seem to hide just the information I want to display to the requesting party per row. The email receives show to much data. I need this information on a monthly basis and I'd really like to automate it. Any suggestions would be appreciated.

I believe this requires you to change the column type to a contact-like one.
Then you should be able to select it in this dropdown.

I believe you can just include specific fields in the request for update -

@Ryan Kramer Thanks, but I'm getting stuck on a couple of spots.
First its only allowing me to choose the following contact:
and the email received duplicates\shows multiple records: I chose to request: Job ##, phase code and bus mileage - seen below.
