Can I put a table in a form?

Is there a way to put a table in a form for the user to complete the table specific content? I'm trying to have the ability for an employee to submit a request related to particular stock and fill out a table in the form that includes the sku #, sku description, qty needed, qty on hand, and date qty on hand was taken.

There currently isn't a way to embed a table in a Form; each of your fields (sku, qty, etc) would need to be listed as separate fields down the form.
