URL query string to prefill default values on a form malfunction. What am I doing wrong?

I'm using a URL query string to prefill default values on a form. When I click the link, it takes me to the form and all of the fields are filled out as expected. However, when I fill out the remainder of the information the form gives me a "invalid or incomplete" error and will not submit. I can submit a blank form with the same information in it, but it will not submit from the URL query string.

Anyone have any experience with this error? I'm not sure what I've done wrong to get the error and the form will not tell me what is invalid or incomplete. Thanks for any guidance you may have.

Do you have validation turned on for any field?
