Attach picture to the Form

Hi All, I am trying to recreate an office supplies order form in Smartsheet. Is there a way to attach pictures to the form as shown below? Thanks!

There currently isn't a way to embed an image within a Smartsheet form as part of the form itself. Please feel free to add your vote and voice to this Product Idea, here: Adding Images in Form Body and Descriptions

As an alternative, you could use a Google Form with images and have that form submission go to an underlying Smartsheet sheet. Here's more information: Smartsheet Sync Add-on for Google Forms
