I need to figure out to apply the Excel VLOOK formula below to a Smartsheet

This Formula is being used on an event tracker and is and I need to understnd how to create the same condition in Smartsheet

Thank you Paul. I will try to find some tie today to work through this. I may have additional questions. Just skimming through this, it sounds like I need to create a second sheet to look up the values. Is that correct?
