Widen the width of the form?

The default form is very narrow, so my form is very long and doesn't allow for easy review of what was answered above-- is there any way to widen it or allow for two columns for form questions?

Most of the other columns in my sheet only require the counting of days from a date in a different column. When the source column in the formula is not filled out the cell is blank.
