Conditional logic not working in my form

Hi,

I have a form that utilizes conditional logic. We have been using this form for a couple years with no changes, and all the sudden the logic stopped working. Here are some screenshots:

In this one, when EUS/US is selected, it should show call ID, which it is doing. But when the other two options are selected, the Call ID field does not disappear:

Second, when passport stamp issued is set to NO, then none of the options below should show up:

Here is what it looks like in the live form:

Finally, the option for someone to send email confirmation is checked, but the option does not show up on the sheet:

That's a lot of issues for one form how unfortunate!

I would try deleting the form and rebuilding it. If it's massive that may not be a good solution, but if you are looking for a quick fix it might do the trick.
