creating a requisition form with automated approval

I am wanting to create a basic requisition form with automated approval, but cannot seem to figure out how. When I create a form, I want people to be able to submit more than one item on a request. For example, if we need office supplies, I want people to be able to put on the form "pens, copy paper, envelopes, staples" and each of those show up on completely separate line items, not just in one cell. Can someone please assist me?
