Help with Forms

I am trying to figure out the best way to set up a form or sheet. I need learners to fill out a form at 30, 60, and 90 days. Each form will have different questions, but I would like all the data that the learners submit to be on one sheet and in one row associated with each learner's name. Is there a way to make a single form that learners can fill out each time and not make a new line for every submission they do, or what options do I have?

I recommend either starting the process with a form and then updating requests or using update requests only.

Then everything is collected on the same row.
