Duplicate sheets showing up on report

I am creating a reporting and selecting multiple sheets for it. I selected 4 sheets but one sheet keeps duplicating. Iv'e attached an image. Any help would be appreciated.

    @tim_sims23 I would guess that the "PM Feedback - to - Project Team" sheet has 2 blank rows on it.

    If you want the report to only pull in one row per sheet, you might consider adding an Auto Number column to each sheet and then filtering your report to only pull in rows where that column equals 1, or something like that.
