I found this template online, but can't find it in SMARTSHEET. Also, I am on the 30 day trial, but do not see where I can import word documents. The import option does not allow for word docs

Smartsheet doesn't give you an option to import word documents, even if you're in a paid plan. To get the template, you will need to be logged in to Smartsheet and click on the Smartsheet link in the template page link that you've shared. A workspace will be created with the details as a sheet, which has a form as well.
