The date on my grid is not what was submitted on the form
I have a sheet that tracks Personnel actions. On this grid, there is a "request date" and "effective date". Suddenly, every action request that came to the grid had the same date, ie: all requests were on 2/1 and all effective dates were 2/6 even though we're in March. I thought the managers were choosing the cached dates on their forms, so I watched one enter his correct dates and when I checked the grid, they were different.
Has anyone experienced this before? We've used this form for almost 2 years with no issue. This started roughly a month ago,

Strange! Have you checked the Activity Log / History Log for clues?

I have, and aside from the automation (copying to another sheet) and the docusign status changing as the forms are signed, there isn't anything being changed.
