Autofill Form Title on sheet

I have a sheet with 3 forms and I'd like the title of the form to show on the sheet when the information is submitted. I prefer that this be autofilled and not visible to those completing the sheet. Is this possible?

You can do this by adding a hidden field to your form.

• Add the form title column to your sheet.
• Add the form title field to the form.
• In the menu on the right set this to be "hidden" (see 1 below).
• Add a default value which is the title of the form (see 2 below).
