Need Help with a Formula for Checked Boxes and Forms

Hello Smartsheets Community.

I'm trying to find the best formula for a checkbox to automatically checkmark once a form is submitted.

Please see below.

I have 5 Questionnaire Forms.

Then I have this sheet tracker

The Status Formula will already change to Completed and Green once the Submitted or N/A Checkmark is checked. I just need a formula to identify when a questionnaire form is submitted.

I was thinking maybe the IFERROR/VLOOKUP?

Formula Column 
= IF(Submitted@row = 1, "Yes", "No")

Replace YES/NO with what you want to show when Submitted Row is checked.
