If/And Formula Help Needed

Hi-I'm new to SS and I need help with a formula. I created a checkbox column that I want to auto flag if two conditions are met. If Provider is typed in the Department Column and if the Date Sent column is greater than 14 days days ago. Can someone please help?

=IF(AND([Department]@row = "Provider", [Date Sent]@row >= TODAY(-14)), 1, 0)
