Pre-populating sheet to show in a form to collect additional information



I am hoping that someone can help me.

I'm creating a form to collect additional information on projects being submitted.

Specifically, I am trying to create the following:

  1. A form was used to submit a project request into a Project Request sheet
  2. Another sheet (Project Information) automatically pulls in a few information from the Project Request sheet (Project Name, Submitter, etc.)
  3. A form from Project Information sheet pre-populated with the Project Name, Submitter, etc. and with additional fields that the user will have to enter for additional information
  4. This form would then be submitted and the information populated into the corresponding rows for Project Name, submitter, etc.
