Forms to Sheet
I have a sheet, loaded with our employees, into which I am trying to populate contact phone numbers using Forms. How can we get the user completing the Form and the contact info they share, to feed back to their corresponding name on the sheet list?

If you already have employees loaded into a sheet you'll want to use aan "Update Request" to gather information from the employee in the same row. A form will always add a new row. The update rerequest Looks like a form but will update info in a row that already exists.
