Document generation error on creation page

I have a template I've used for a few years that all of a sudden will generate and download, but not generate and attach to the row (as I need it to).

Is there a setting I need to change? It just sits there and shows it's attempting it (I've waited 12 minutes for the last one), but the download one will complete itself in 2-4 seconds. What am I missing?

Are you utilizing the Automation Workflows feature in the sheet to generate the document? If so, are you willing to attach a screenshot of the workflow settings? It's possible that the triggers for the workflow are causing some sort of error.

Does it work sometimes and other times not?

This is a manual function, not automated - we will do 1-30 documents at once. Two people tested this on single and multiple rows and it is not working for either of us.
