using form on multiple web pages
I have two websites which we'd like to use the same form on. Would like to have a field when a form is submitted that automatically fills in on the spreadsheet which webpage or name the form details came from. EX. COM or Frey. Is this possible? the field would be hidden on the form of course.

Hi, @LorryH , another approach is to create two forms for the same sheet: "COM" and "Frey". In the field where you plan to store the name of the site, set the default value to the site name. Set the field to hidden.

Perhaps have the form attached to a third placeholder sheet that feeds the other 2 sheets? You could include a column that has the names of the sites you're working with and based on the submitted report values a workflow could copy the new row from the placeholder to the correct sheet. The sites column could be a hidden field on the form that get's set by the link you provide. The link to the form would look something like this with Website being the name of the hidden form field that will trigger the workflow in the placeholder sheet when submitted:

Thank you both, the first link didn't work. I did setup two forms, one for COM, one for Frey, linked to the same spreadsheet. Ran a test which works, thanks.
