Has anyone used Smartsheet to create a User Request Form or User Access Requests Form?

Typically, URFs or UARs have been paper-based. I am wondering if anyone has created and successfully implemented a system to not only track the overall requests but to alert the various people across the organization if they are assigned actions based on the system(s) they oversee.

Quality Administration Request - Intake.xlsx

This is an excel spreadsheet of some of what we have in Smartsheet. I removed any column names and formulas that were not generic. I included formulas that we use for some reporting and to assign our tasks. We have automations set up based on the QA admins email address for notifications.
