Has anyone used Smartsheet to create a User Request Form or User Access Requests Form?

Typically, URFs or UARs have been paper-based. I am wondering if anyone has created and successfully implemented a system to not only track the overall requests but to alert the various people across the organization if they are assigned actions based on the system(s) they oversee.
