User receiving large number of notifications from workflow

I am experiencing this problem, however Smartsheet sent out 210 notifications to single email recipient. He should only need to get 2, one for the form submission and one for approval.

Did you check the activity log to see if there were multiple changes made to the sheet that would have triggered it this many times?
