Prefill a form response from another sheet

I have a project tracker sheet and am trying to set up a flow to send a survey via a form once the status of the project is changed to complete. I would like the survey to prefill with the project name from the source sheet, of the project that was changed to closed. Is this possible? The form results (survey results) could fill to the current or a seperate sheet.

Unfortunately data only flows one-way on Forms. Forms only input data into your sheet, and cannot pull data from your sheet.
