File Upload Form Element grayed out, how to fix?

I'm trying to adda a File Upload form element but the box is grayed out, is there a solution for this issue?

@M N,
The only way I know that the option can be grayed out is if it's already been added to the Form.
It can only be added once.
Did you by chance already add it?
