White Box when submitting a Smartsheet Form

I have created a request form for users at my organization to submit requests. On multiple occasions, users have attempted to submit the form and get a white box over the screen. It does not advance any further and does not submit. What is happening?

this is a very odd behaviour that I have never seen before.

does it happen for all the submitters of the form? It could have something to do with the Browser - did you test it on different browsers or inside the mobile app / or desktop app?

It has happened to all users on various desktop browsers. I am assuming it's a timeout feature, but I could be wrong. Members on our team are having to re-enter the form. It usually goes through the second time.

It looks like a typical action where the Smartsheet Support needs to be involved. Do you know how to open the ticket with them?
