Form with multiple logic conditions

I have a form with multiple logic conditions (15). The logic is that who fills the form is being asked how many items he/she wants to insert/record. If 1 is chosen, the form will show only one field (Item 1) and if 2, two fields will show (Item 1 and Item 3) and so on. No matter how items (1 to 15) are chosen, the Form should show that number of Item fields but always after the last Item field, 3 other field must show on the form before the Submit button. The problem I have is that all shows fine until (and including) the Item 5. When I choose 6 Items, the last 3 fields (and the "Submit button) are not showing anymore. Does anyone have any idea how to fix this? Thank you!

UPDATE (2 mins later): It was probably just a background Refresh that Smartsheet did and all was fine. Thank you, anyway for those that I am sure would jump in to help me! I hope I get a rain check for next time!! 🤣
