Can I add an image uploaded to a row via a form to a PDF in Generate Documents?

I have created a mobile "model release" for photography. I present the release language and record the subjects information using a form. They can capture and upload a reference photo. I'd like that photo to be placed no the resulting PDF. Can I map file uploads to the PDF?

Also, how can I get the name of the resulting PDF to not be the same name every single time?? I'd prefer something like 20250312-John Doe.pdf

unfortunately as for now the images cant be transferred to pdf's as parto of document builder :(

check if this feature is not requested already in the new product ideas section and vote for it if it's already there or create new post requesting it.
