Charting Problem

I am using Smartsheet to track the probationary period of new paramedics. One of the things I need to track is skills success rates. Currently, the medic completes a form for each patient treated. Two of the items are Total Number of IV attempts and Number of Successful IV Attempts. All medics go to the same sheet. I created individual reports for each medic for each skill tracked. Unfortunately, when I create a chart from that report It will not give me totals. It simply creates a bar or wedge for each patient encounter reported (see below)

. How would I go about getting it to create one wedge with the total number of successes and total misses? Plus, a percentage on the side.

is it possible for you to give screenshot of data from sheet you are sharing not only the chart? also how you set up the chart? it would be easier to diagnose it.
