Smartsheet Form QR Code

I created a QR Code using a Smartsheet Form URL; however, the QR Code takes me to a Smartsheet sign-up page instead of the form. I tested this with several QR Code generators. Any idea of what's happening here?

Clearing the cookies in my browsers fixed this issue.

Did you use the "back end" form url (accessing the form from a workspace while logged in to Smartsheet) or a web url that doesn't require a login to access?

Also, what happens when you go through the steps of "I already have an account" ? Does logging in then redirect you to the form?

I used a web url that doesn't require a login. When I go through the steps of "I already have an account," The form displays, once I login.
