Keeping a submitted form as a document

hi there,

I'm setting up a new Smartsheet - i also need a form to go with it. usually the form gets submitted and a response at the end says captured. i would like to keep a copy of the actual form as a document - to download it. Is this possible, if so, how do I go about this? thanks so much for any help.
