The form allows to check the box:
However, when checking the box the form remains inactive, no error message appears. Form does not complete.
Only by unchecking the box will the form complete/submit. Please advise of fix. Thank you!

Checking that box should then display a field for the user to enter their email address. The form then won't be submittable until the email address is entered. Can you check that the "enter email address" field isn't appearing on a non-visible part of the screen, possibly scroll down.
