Web Form- Being able to include a image in a form
Just wanted to toss this out for consideration. We use the web forms quite a bit, and for some applications it would be very helpful for us to be able to include an image as a field in the form when you build the form. For example, we could have a picture of the back of their ID's and circle where to find the 4 digit code that we need for them to populate into the web form. So when they get to that question on the form they already have the image in front of them showing them where to find the number.

Yes, this could be very helpful. I recommend submitting a Product Enhancement Request.

Additionally, you can use a third party form site such as google forms:
https://www.smartsheet.com/apps/google-forms-smartsheet-sync
This can add information to your smartsheet, with a few more features than the SS forms.
