Attachment in an online Form

Unfortunately, it's not possible at the moment to have an attachment in the form, but it's an excellent idea!

Please submit an Enhancement Request when you have a moment

As a possible workaround, you could add a link to the attachment instead.

Would that work?

Lol the "Please submit an Enhancement Request" form you posted has an area to submit attachments to it - so it's obviously possible.
