I've created a form with notification delivery, I know i can edit the columns to the recipient. Is there a way to only have items selected in the Form that the user has selected to be sent in the notification message?

I am trying to create a Purchase Order in smartsheet. I looked at the community and an old thread said there was a purchase request template, but i guess it is gone because I do not have it? Is there a good way to create purchase orders within smartsheet?
