Unfortunately, it's not possible natively inside Smartsheet, but you can do it with a third party solution like Google Forms or similar.
A possible workaround could be to have two dropdown lists.

You will have to separate it into two different columns which will provide two different fields on the form. Two options cannot be selected within the same dropdown on a form.
