Unfortunately, it's not possible natively inside Smartsheet, but you can do it with a third party solution like Google Forms or similar.

There is currently no conditional logic within forms. Either the question is there or it isn't. Unfortunately you currently cannot have new fields generated based on a selection within a form.

Today I set up an automated alert when rows are added or changed, but when I get the notification email the specific changes are no longer highlighted as they were before (as recently as last week). Has something changed? Has anyone else experienced this problem?
