I actually want the whole page/form to appear as an independent form/page when we visit one of our subdomain URL.
To embed the form, you would use an iframe HTML code. Isn't that what you want to do?

I do not want to embed this to another HTML page. I just want to link the form to a subdomain on our page so that when I visit form.mydomain.com, the Smartsheet form opens.

Ok. Then you would only need to link that domain to the form link.

I have tried to set them as Edit - can share, Edit - cannot share, and commenter and nothing works, they cannot add data to the sheet. Does anyone have a solution?
