I can think of a few ways to set this up. Does it need to be anonymous?
If not, it could work to make an update request instead.

Yes it does need to be anonymous. It will also be sent to employees who do not use smartsheet at our organization. If it was sent via an update request, would all employees need to be entered as a contact?

No, to send the UR you only need the e-mail addresses. You could hide the e-mails and delete them after the answer has been submitted.

As a new user, I have the same question. I don't understand the process Andree is suggesting, and the attachments didn't make it any clearer for me.
I'd like to create a form in which an employee can only submit their response one time. They would not be able to return to the form and make a correction or duplicate entry.

I had a new idea! (untested, but should work)
We could use the automatic move row function.
If there is already a submission from the employee, it will move the row to another sheet.
Would that work?
