Mutiple Contacts / Update Request / Form

Hi All, 

I have a column that has multiple contacts. I'd like to send an update request or form to each contact in the cell but I don't want one contact to overwrite another contact's information. I want to capture the information from ALL contacts in the cell.  

Questions:

1) Form or Update Request?

2) Is it possible to have each contact's entered information stay without being overridden?

This is for meeting attendance.  Each contact has attended a meeting and we'd like to gather post-meeting feedback.  There are some meetings that have 1-5 attendees and we'd like each of them to provide feedback.  Hope this makes sense.

There are several ways to set this up. Here are a few examples!

1. All can write in the same cell, but they could then delete/change something that's already there.
2. All could have their specific cell that they could write in, but if it's one Alert & Action, they could delete/change other cells. (If it's one rule per person where it's set up only to show that specific persons cell it would work, but it's more cumbersome. Those cells could then be joined in another cell.
3. All could add their feedback as comments instead and then everything would be in one place.
4. A form could be sent to gather the feedback, and the answers would be on one row for each answer.

Which method(s) do you prefer?
