I created a workflow based on date but it is not sending updates as expected
I created the following workflow and I cannot seem to figure out why it is not sending the updates as expected.
The row that should have triggered it is below:
I can't seem to figure out what I have set wrong here. I just want it to send the rows that have today as the upgrade date to the people listed in the contacts at the bottom (all are members of my organization and have the sheet shared to them.
Anyone have any ideas?

I should add that the Req Upgrade Date column is pull down and populated by a form users submit. I had to use pull down to limit the dates they could select. In order to get it into a date format column I have a formula that takes the date that was submitted by the form and puts it into the date format column Upgrade Date. Could the problem be the formula I am using to pull the date over?
If so, is there a way I can limit input dates on a form so I don't have to have the formula move the date over?

Further note:
I just went back and checked the values on the pick list, as it turns out they were not in the format that the date column would use so I changed the choices to be an exact match to what would have populated from the calendar. After checking further I feel like copying the date from the text column to the date format column may be the issue. I suspect it is inserting the date into the date column in text format which I think might be causing it to not trigger.

I figured it out, it was the format that I was using in the pull down list.
Once I updated the list options to the format that the calendar picker uses, it started working.

Well now there is further complication. The automation worked once. I have changed absolutely nothing and now it has stopped working again.

I can send you columns and test rows but all information in this sheet is proprietary.

I sanitized all the proprietary information leaving just the columns and information I am using.
Keep in mind the Upgrade Date is the column that triggers the workflow. The contents of that column are copied from the Req Upgrade Date column. I had to do this because you cannot restrict date choices in a date column on a form in Smartsheet. I did not want to allow the form submitters to choose a date out of the project range. I suspect the fact that I am doing that is what is causing the problem. Strange that it worked once though.
