Do you have a template that could be used to manage supply chain risk? What template could be used in order to properly manage it?

We have a number of templates available to download in our Solution Center (see here).
If you search the word "Risk" a few different options will pop up, including one called a "Risk Assessment Matrix" that you may find helpful.

Report Builder Query - Grouping
I have created a report, but for some reason I am unable to group by the Assigned To field, which is not displayed in the drop-down options, even though the field is displayed in the report. What am I doing wrong?
Modified by automation instead of name
Good afternoon, If I send a request for info and there is a check box, if they click on the box via the request in the email, it says it's modified by: automation@smartsheet.com instead of Bob. Is there a way to get the person who clicked on the boxes name on there instead of automation? Thanks
