I have one task to create the approval and form . The story as below :

1. I have a project with the due date and result. And if any project member want to extend the due date, they need a approval from manager. If the manager accept, the due date with change to new due date. If he decline, nothing change. So how can i do ?
2. Can using a Form in smartsheet as the approval form, and after submit, it will send to Manager to approve ? So can you help me this one. Thanks

1. Unfortunately, it's not possible now to have the value change after Approval, but it's an excellent idea!

Please submit an Enhancement Request when you have a moment

As a possible workaround, you could use a 3rd party service like Zapier or similar. Is that an option?

2. Yes, the form could trigger an Approval workflow. Can you describe your process in more detail?
