Gaps in rows

we recently started using smart sheet for our inspections, now i observed that once the inspection is submitted. in smart sheet it is not getting posted in the next row, dont know why there are few rows are left in between?. can anyone tell me why?

here it is we did couple of them as test and later on they are getting added with gaps.

Are you adding the form submissions at the top or bottom of the sheet?

NO today it left few more rows, here is the photo. blank cells are not consistent. sorry for delay i just saw your comment.
